Account Set-up Instructions

When setting up an account with Sunwest you will fill out four (4) forms. Each form can be filled in completely on-line then printed and signed before sending to Sunwest Trust. The needed documents are as follows:

Custodial Account Application

This form is several pages long, only the first page of the application needs to be sent to Sunwest. The remainder of the document is your plan agreement as required by the IRS.

The custodial information should already be filled out with Sunwest’s information.

Depositor Information: This information is pretty straight forward, your name and address information. Every blank except the Plan No should be filled out. When we receive the application we will assign a Plan No.

Designation of Beneficiary

This is where you tell Sunwest who gets your IRA upon your death. The first box that you can check tells Sunwest that if you have more that one Beneficiary and one of them dies the entire amount goes to the other named beneficiary.

The second box tells Sunwest that the interest of the deceased beneficiary goes to his or her heirs and that the interest going to the remaining named beneficiary stays the same.

There is room for two Primary Beneficiaries but you can have as many as you want, simply use a blank piece of paper and give the same information on each additional beneficiary. The total interest of all beneficiaries cannot exceed 100%.

If you want to name secondary beneficiaries you may do so on an additional paper attached to your application. Make sure you designate them as secondary beneficiaries above their names.

All blanks must be filled in including Social Security Numbers (SSN) and date of birth. Again the share % for all beneficiaries must equal 100%.

At the bottom of the left column the applicant must sign and date as depositor. Below that Sunwest will sign and date when we receive the application.

The Witness Signature is only needed if you have a spouse and they are not named as 100% share beneficiary.

The top right hand column Deposit Information is only used if you are making a contribution at the time you are opening this account. If you are transferring an existing account you will not need to fill this information in.

Type of Contribution: In most cases you will check the box next to Transfer – From Another IRA Custodian to: Regular. If you don’t think this is correct for your situation call Sunwest to determine which box you should check.

Financial Disclosure: This section is already checked because you are opening a self-directed account and Sunwest has no way to determine what kind of financial return you will get.

Bottom right hand column: If your spouse is not 100% share beneficiary of your IRA he or she must sign and have their signature witnessed.

This document simply needs to be read, signed and sent to Sunwest.

Sunwest Trust, Inc. Self-Directed Account Holder Disclosure And Hold Harmless

This is a very important document that needs to be read and if needed reviewed with your tax accountant or attorney. This document explains your responsibility in having a self-directed IRA including prohibited transactions and disqualified parties. It is your responsibility to monitor your own account and avoid those transactions that might cause your IRA to be disqualified by the IRS.

This requires only basic information and a signature on the last page.

Instruction & Authorization to Transfer IRA Funds

This is the document that will be sent to your current custodian to initiate the transfer.

Top right requires information about your current custodian and next to that your account information. Most if not all of this information can be obtained from your last account statement from your current custodian.

Section I. is where you tell your current custodian how you want them to transfer assets, be it cash or “in kind”. You can instruct them to transfer all or only a part of your current account either by liquidating your current assets and transferring cash or by transferring assets “in kind”.

Pay close attention below if you are over 701/2 and are subject to RMD and check that box that applies to your situation. Then sign below and mail this form along with the others described above to Sunwest and we will do the rest.

Section II should already be filled out with Sunwest Trust Inc. information and will be signed when we receive it.

Section III is for your current custodian.

This completes the application process, simply make copies for your records and mail the originals to Sunwest Trust, Inc.
