STEP 1) Please complete and print all the following forms
> IRA Custodial Account Application and Plan Agreement
(return the first page of the application and retain the Plan Agreement for your records)
> Fee Disclosure
> Account Holder Hold Harmless
(please return all 5 pages)
> Transfer Form from another IRA custodian
> Direction of Investment (if applicable)
STEP 2) Sign and retain a copy of the above forms for your records
STEP 3) In accordance with the USA Patriot Act, please also include a clear copy of your drivers license. Please note that no investments or distributions will be made until this is received.
